Introduction
Where physicians are involved in case management, they coordinate such elements
as referrals to consultants, specialists, hospitals, ancillary providers and services. This
is intended to eliminate inappropriate utilisation of facilities and resources,
fragmented services and to provide continuity of services and intensity of services
appropriate to the patient's needs over time.
Case management involves delivering care across traditional professional
boundaries. This means that care teams must work together as doctors and nurses,
specialist doctors and primary care doctors, and other combinations of clinical
caregivers. Turf battles and power struggles between and within professional groups
must be broken down to enable collaboration for the patient's best interest.
Therefore, managed care is the broad coordination of patient care and collaboration
with various care providers to ensure the appropriateness of the treatment and the
cost thereof. This includes peer reviews and utilisation reviews of drugs and
equipment, facilities and other resources such as laboratory tests
